Aging with Dignity and Independence - HealthyCal.org

By Victoria R. Ballesteros and Athan G. Bezaitis

Aging with dignity and independence is the ability to live life to its fullest in the place you call home regardless of age, illness or disability. While we all like to picture ourselves growing older in a healthy way, the reality is that 70 percent of people over the age of 65 will need help with daily activities at some point in their lives ? for an average of three years. This care can touch every aspect of your life ? from how you live to where you live ? and it can be very costly. Unfortunately, you never know when you or someone you love might need daily help, such as assistance getting groceries, help with transportation, or round the clock care, all of which require planning and coordination.

The first step is to build a plan that preserves your choices, dignity and independence, even when you need some help to do so. The second step is to start conversations with the people around you ? family and friends, health care and social service providers, legal and financial experts, and others ? about what you want and what is important to you in case the need arises.

The SCAN Foundation recently produced a guide featuring 10 key areas with conversation starters that can help you begin planning for the future you want as you grow older.

?Personal Values?

1. The most important things in your life that define who you are

Consider the activities in life that are most significant to you, such as spending time with your loved ones or your favorite hobbies. Think about your personal or cultural preferences, such as foods you like to eat, the language that is easiest for you and religious or spiritual beliefs.

Suggested conversation starters:

The things that are most important to me are?

My favorite everyday activities are?

The language that I am most comfortable with is?

Beliefs that are important to me are?

2. Your legacy of values

Think about how you want to be remembered in years to come by loved ones and future generations. Consider your life?s lessons, values, hopes and dreams for the future. Recording these thoughts with the intent to share them with your loved ones is called an ?ethical will.?

Though not a legal document, an ethical will can help you to put your life into perspective and share your future hopes and dreams.

Suggested conversation starters:

I?m most proud of having done?

The most powerful lessons I have learned so far are?

My goals for the future are to?

I plan to do these things by?

?Day-To-Day Considerations?

3. Where you wish to live

Consider whether or not your current home is the best place for you over time. If you live in a two-story home, it might be important to have a bathroom and bedroom on the first floor. There may be hazards such as dim lighting or narrow doorways that, if addressed, may make it easier to get around. Think about the size of your home and the required upkeep, along with any changes that can be made to suit you better as your needs may change. Also, consider whether you will have the option to live with other family members or friends if help is needed.

Suggested conversation starters:

I do/do not want to stay in my current home because?

My home may need modifications in order for me to stay over time such as?

Living in another place, such as an apartment or senior community may be more appropriate for my needs because?

If I cannot live safe alone, I would like to live with?

4. Key contacts and important information

Key contacts include family members and friends, medical professionals, insurance agents, financial planners, accountants and lawyers who should be notified if you need care. Think about a secure location to keep the important documents associated with these individuals, such as a safe place in your bedroom or home office, and then tell a trusted person where to find them. You may also want to put a list of important telephone numbers and email addresses in a central location, like the refrigerator, and be sure to keep them up-to-date.

Suggested conversation starters:

I have put together a list of key contacts that includes the following people?

I have compiled copies of the following important legal, medical, and financial documents?

These important documents can be found in?

?Health and Well-Being?

5. A nutritious diet

Ensuring that you have access to nutritious food ? whether from grocery stores and nearby restaurants or neighbors, friends and family ? is important. Consider how often you cook for yourself, eat out or order-in, and how your access to healthy meals might be affected if you could not easily leave your house for a period of time. Look into the availability of programs that can help, such as home-delivery for meals and groceries. Consider people on your list of key contacts described above who could help with this task.

Suggested conversation starters:

If I could not leave my house, I would get groceries or regular meals by?

Local programs and services available for food and groceries are?

Family members, friends, and neighbors who could help me get regular meals are?

6. Health and Well-Being

Being ?healthy? encompasses more than managing a medical diagnosis. It also includes your emotional health and social activities, which can affect your ability to do the things you enjoy most. So while it is important to keep regular check-ups with your physician and to track your medications ? both prescription and over-the-counter ? think also about physical and social activities that you would like to continue doing over time.

Suggested conversation starters:

My main doctor who knows the most about me is?

The last time I had a physical examination was?

I need to? (make appointments for the following medical tests?) or (monitor the following conditions?) or (take the following medications)?

My favorite physical and social activities are?

One step I could take to become healthier is?

If I need someone to talk to, I can call?

?Health Care Decisions?

7. A surrogate decision-maker

It is important to identify a person you trust for the role of making medical decisions on your behalf in case you are not able to do so. Through a legal document known as a Durable Power of Attorney for Healthcare Decisions, you can designate this person to make health care decisions on your behalf, should the need arise. Talk to the person that you designate for this role. Ask them if they are comfortable with the responsibility, and then let them know your wishes for health care. Let others know that you have chosen that person to be the ?point person? for your health care decisions.

Suggested conversation starters:

If I am unable to make important health care decisions on my own, I would like you to
make them on my behalf because?

I have completed a Durable Power of Attorney for Health Care Decisions and here is a copy for you. I have my own copy saved in this location?

Let me share with you the health care decisions that are important to me if I cannot make them on my own? (see below for more on this topic)

8. Determine the care that you want for yourself

Most people have thought about what they would want for treatment if they have a medical emergency. Consider preparing a living will, also known as an advance directive, which is a legal document to state your wishes regarding the care you desire if you cannot communicate these decisions. This is your opportunity to decide on maintaining a certain level of comfort while also considering the degree of medical intervention that is important to you. Other legal documents that can help express your desires are a Physician?s Orders for Life Sustaining Treatment form, also known as a POLST form, or the Five Wishes form, which can help medical professionals honor your preferred health care decisions. Find out which of these is recognized where you live as a legally binding document. Be sure to share it with your surrogate decision-maker as described above.

Suggested conversation starters:

If I have a medical emergency, it is important to me that?

I would like your help to complete a document that makes these wishes known in case I am ever unable to communicate my own health care decisions. I understand there are different legal forms depending on where I live. Can you help me with finding the right one?

Can you help me fill out a? (advance directive, POLST, Five Wishes) document so that my wishes are known?

9. Be informed about how to access help in your community

Many communities have services that help people who have chronic illness or disability to meet their personal or health needs on a daily basis. Often called ?long-term services and supports,? this kind of help can assist people with daily activities, such as dressing, bathing and cooking. Round-the-clock care and nursing homes are also part of this spectrum of help, but come at a significant cost and are NOT covered by Medicare. Start with your local Area Agency on Aging, which provides information about and coordinates community services for seniors and adults with disabilities. Also consider whether purchasing a long-term care insurance policy is right for you given your budget and the cost of care. Know your options to address your care needs should the situation arise that you someday need this kind of support.

Suggested conversation starters:

For expert advice with care planning, I can talk with?

My local Area Agency on Aging informs me of the following options?

I have determined that purchasing a long-term care insurance policy is or is not the right product for me because?

?Financial Considerations?

10. Financial considerations

Think about what it would cost to maintain your current standard of living if you need expensive long-term services and supports. It is important to understand your assets and income, current debt, spending habits, available liquid assets, and plans to cover the costs for medical and daily personal assistance. Also consider how you would like to distribute your assets, such as support for your grandchildren or a bequest to a worthwhile charity if you are able to do so. Share these intentions with those closest to you, and discuss how you plan to reach these objectives.

Suggested conversation starters:

The money I have saved for health care and daily living support is?

My monthly income is?

The money I spend each month is?

My debt is?

My will or testament dictates the following?

These conversation starters can help guide you through the process of being prepared for future needs as you grow older. They can also help prevent you and your loved ones from having to make hasty decisions in a time of crisis. Thinking about your wishes and communicating them to your loved ones, health care providers and other appropriate professionals is an important step towards aging with dignity and independence.

(AP) ? This is a little awkward.

President Barack Obama can't seem to stop bad-mouthing the record of former President George W. Bush. But on Thursday, Obama is going to welcome his predecessor and proudly preside as Bush's image and legacy are enshrined at the White House forever.

Obama and first lady Michelle Obama will join Bush and his wife, Laura, as their official portraits are unveiled. The incumbent is keeping up a presidential tradition typically defined by cheer and graciousness, but not without some uneasiness.

Hardly a day goes by without Obama or his aides talking about the mess they inherited ? meaning, from Bush.

It was just one week ago that Obama, revving up campaign donors, turned Bush into a punch line. Obama depicted Republican rival Mitt Romney as a peddler of bad economic ideas, helping the rich at the expense of the middle class, and then added to laughs: "That was tried, remember? The last guy did all this."

Now the last guy is coming back.

So, too, will his father, former President George H.W. Bush and the former first lady Barbara Bush. The Obamas will hold forth in the ornate East Room as George and Laura Bush are honored for their service before an invited audience of Bush friends and former staff members.

It will be a rare limelight moment for Bush, who has not been back in more than two years.

Obama and Bush have a cordial and respectful relationship, but they are not close. Both are political veterans who are able to separate political tactics from what they see as an overarching community among people who have served in the Oval Office, according to people close to them.

Only 44 men in history, and five men alive, have held the job.

"President Bush has been around politics a long time. He's been around how presidents deal with each other for a long time," said Tony Fratto, one of his former spokesmen at the White House. "He has an understanding for separating the necessities of political rhetoric from the job itself."

Bush showed that all through 2008, when Obama assailed his record on war and the economy en route to the White House. It was hard to remember at times that Obama was running not against Bush, who was finishing the last year of a tumultuous eight-year term, but rather Arizona Sen. John McCain.

When it was done, Bush welcomed Obama to the White House with grace and demanded that his team ensure a smooth transition.

History has marked this moment before, with grudges put aside.

When Bill Clinton came back for his portrait unveiling, Bush lauded him for "the forward-looking spirit that Americans like in a president." Never mind that Bush had run for the presidency to "restore honor and dignity" after Clinton's sex scandal.

And when Clinton welcomed back George H.W. Bush, whom he had defeated, he said to him and his wife: "Welcome home. We're glad to have you here."

"I would be surprised if there's very much tension" this time around, said Cal Jillson, a political science professor at Southern Methodist University who has long followed Bush's career.

Obama has enlisted Bush's help on earthquake relief for Haiti, and the two stood together in New York City last year in marking the 10th anniversary of the Sept. 11 attacks on America. They have also spoken at least three times at signature moments over the last three years, including the killing of Osama bin Laden.

Still, in the midst of a tight election year, the Obama-bashing-Bush's-record sets a backdrop.

"This president is looking for someone to blame," Romney said while campaigning in Colorado this week. "Of course, he started off by blaming George Bush, and that worked for a while but, you know, after three and a half years that wears kind of thin."

The White House points out that Obama praises Bush sometimes, too, as he did in March over Bush's willingness to take on immigration.

The visit is layered with political story lines.

Bush's brother Jeb is a potential vice presidential candidate to Romney. Bush's father has developed a kinship of sorts with Obama. And then there is Bush himself, who has endorsed Romney but is still viewed by many in his party as politically toxic.

More than any president in recent memory, Bush has not just intentionally faded from the public spotlight but all but disappeared from it.

"George W. Bush has been remarkably, and even strangely silent, even once you respect his sentiment that he did not want to get in Barack Obama's way," said Jillson. "I think part of that is just giving himself time to recover from what had to be an astoundingly difficult close to his presidency."

The politically impassioned issues of that time have faded. The Iraq war is over. The financial sector has stabilized after a devastating crash in late 2008. But the nation is still feeling the cost of the enormous recession, which is Obama's problem now.

Bush was last at the White House in January 2010. That was to join Obama and Bill Clinton in support of Haiti humanitarian relief.

Aides to both Obama and Bush are downplaying the Thursday reunion as a time of politics. Bush spokesman Freddy Ford said the former president and first lady are grateful to the Obama and looking forward to catching up with faces from their past, including staff at the Executive Mansion.

"I think there is a community here with very few members that transcends political and policy differences," White House spokesman Jay Carney said. He made that comment in the same briefing Wednesday in which he reminded everyone that Obama inherited a huge budget deficit (from Bush.)

Jenna Bush Hager, one of the George W. Bush's daughters, said she was invited for the ceremony and that the day will include a private lunch for the Bushes with the Obamas. She told "Fox & Friends" the day will be a chance to "celebrate his work, 'cause he worked pretty hard so I think he deserves at least a painting."

As to where it will go, she said: "Probably in the very back somewhere. I'm just kidding."

The painting will actually hang prominently in the formal entrance hall to the White House, the Grand Foyer.

Considerations in Choosing a Suitable Hotel in Rome

There are many who claim to be one of the top hotels in Rome. While some establishments really do have the quality to justify their claims, there are some for whom the statement is purely lip service. It really is up to the guests to determine if a hotel is good enough to be considered as one of the top hotels in Rome.

Even if a hotel is considered to be one of the top hotels in Rome, it may not be what a particular tourist has in mind. Some people travel on a budget, and as such, they would prefer staying at a cheaper hotel that gives great value for the money they have spent. Whatever a person?s needs are, they are sure to find a hotel that is suitable for them.
Here are a few simple tips to help a person find the top hotels in Rome for tourists.
Consider the rating given to the hotels.

The most definite way to get an idea of a hotel?s facilities and the quality of their service is the rating. Some hotels are rated as four or five-starred. These high class hotels are perfect for guests who want to be pampered and who would like to spend their vacation in luxury. The top hotels in Rome are given this rating. Most five star or four star hotels have luxury accommodations with complete facilities. These hotels have everything, including restaurants that are sure to give their guests a memorable dining experience. The service at these hotels is very attentive as well. A person would expect nothing else from a hotel that was given this high rating. Of course, as can be expected, the rates at these hotels are very high. Those who are on a limited budget may do better to choose other hotels that provide their services at lower costs.

Look for a hotel that is near the sites you want to visit.
Location is of the utmost importance when choosing a hotel to stay in. Many of the top hotels in Rome are located right at the Spanish Steps. Guests staying at these hotels will be able to get to the many of Rome?s best tourist spots in a short amount of time. Also, staying at the city center will allow the tourist to fully experience the ambience of living in one of the most beautiful historical cities in the world.

Factor in the transportation costs.

People who are working with a limited budget often consider the cost as one of the most important factors in choosing a hotel to stay in. However, in their desire to find an affordable hotel, they sometimes end up checking into one that is a bit far from the center. This means that they would have to spend more on public transportation just to get to the center every day. In the end, the total cost of lodging and transportation may come out to be much more expensive than if they had checked into a more expensive hotel in the first place.

Amid bribery scandal, Wal-Mart talks integrity

By Jessica Wohl and Lisa Baertlein, Reuters

FAYETTEVILLE, Arkansas -- ?Top executives of Wal-Mart Stores Inc did not directly mention a Mexican bribery scandal at an employee pep rally on Wednesday, but asked their international workers to focus on "integrity" as a core value.

The world's largest retailer has been under fire from shareholders and activists after the New York Times reported in April that management at Wal-Mart de Mexico, or Walmex, allegedly orchestrated bribes of $24 million to help it grow quickly last decade and that Wal-Mart's top brass tried to cover it up.

The matter is being investigated by a number of government agencies in Mexico and the U.S. Department of Justice and the U.S. Securities and Exchange Commission. Wal-Mart is also conducting an internal probe.

Executives talked around the allegations in Mexico, the company's first and largest international market. They referenced integrity as one of the company's core values and underscored the importance of complying with local laws.

"It's doing the right thing, every single day," Wal-Mart President and Chief Executive Officer Mike Duke told international workers gathered at the University of Arkansas' Barnhill Arena.

Doug McMillon, president and CEO of Walmart International, said it was up to every employee -- from new associates to top executives -- to guard the company's principles.

"If you see something in your business that you don't think is right, you need to say something," McMillon said.

Scot Rank, president and CEO, Walmart Mexico and Central America, may have come the closest to directly on the allegations, without specifically mentioning bribery.

"Over the years we have faced difficult and challenging times in Mexico and Central America. These events have united us even more, they have motivated us to continue, to continue pursuing excellence, and work with respect and integrity," Rank said at the end of the Mexico and Central America unit's presentation.

Wal-Mart's focus in China, the world's fastest-growing major economy, will be on expanding the right way, Scott Price, president and CEO of Walmart Asia, said earlier in the morning.

"We have to be compliant, we have to follow every rule, every regulation in the country," Price said.

Shareholders, including some of the largest U.S. public pension funds, have called for Duke, former Wal-Mart Stores CEO Lee Scott, Chairman Robson "Rob" Walton and others to be ousted from the company's board of directors in the wake of the Mexico scandal. But such efforts have little chance of succeeding because the family of founder Sam Walton controls roughly half of Wal-Mart's shares and votes.

More than 1,300 international employees traveled to northwest Arkansas for this week's events, which culminate in Friday's annual shareholders meeting.

During Wednesday's meeting, as in previous years, a selection of top international employees were given "Integrity in Action" awards.

Despite the bribery allegations, this year's celebration is still expected to be an upbeat affair. Wal-Mart is marking 50 years since Walton opened the first Walmart store on July 2, 1962, and at Wednesday's meeting representatives from Chile played a video of employees in that country singing "Happy Birthday."

Company shares, which have more than rebounded from the roughly 8 percent drop in the days following the New York Times Mexico story, are trading near all-time highs. The stock was up 16 cents at $65.84 at midday on Wednesday after rising to $65.95.
