Signs of Skin Cancer: Deadly Skin Cancer Types

An important warning for us all with regard to skin cancer types. Today more patients than ever survive dangerous melanoma than in past generations, but the rate of death for one form of this disease hasn't budged in 30 years.

Nodular melanoma accounts for 14% of melanomas that are diagnosed, but makes up a startling 37% of untimely deaths from the disease according to a recent study.

The reason this form of cancer contributes to so many deaths is that it doesn't always look like what we expect to see. It can appear without warning on the surface of the skin and can be hard to spot because it's often light or nearly colorless, and doesn't adhere to the ABCD characteristics we've all been taught to look for.

Just like the more well known radial melanoma, nodular melanomas start out in skin cells that are called melanocytes that product melanin. Most melanomas stay in the skin's top layer, going down no more than one tenth of a millimeter. Radial melanomas are easier to see as the diameter goes up.

For nodular melanoma the cancerous cells reach a point where they start to grow vertically, sending roots into the fat and other tissues of the body. The depth of the cancer determines how likely you are to survive. For a cancerous growth that goes 3.5 to 4 millimeters deep, only 60% of patients will live for five years.

This latest study examined over 100,000 cases of melanoma diagnosed between the years 1978 and 2007.

They saw that at least 20% of nodular melanomas were fatal. You should understand that there's an acknowledged limitation of the work - not all physicians report the type of melanoma and the team was unable to double check the reports during the study.

While public health officials and others have done a great job of getting the word out about ABCD characteristics when it comes to identifying skin cancer, experts now believe other letters should be added to the mix to help identify the nodular types of these cancers. We've been taught to watch for any of these...

- Asymmetry, one side doesn't match the other

- Borders, where moles have irregular edges

- Color, color changes or a variety of colors

- Diameter, anything over 6 millimeters wide

But we should also be watching our skin surface for E, F and G as well. Any mole or other growth that has all of these characteristics should be checked by a dermatologist at once. They are...

- Evolving, where the mole or nodule is changing quickly over a short period of time

- Firm to the touch

- Growing for more than two to three weeks

Also good to know, nodular melanoma can happen in a mole, but are more likely to develop without warning on normal skin

In most cases the various skin cancer types, including melanomas are easy to detect, treat and cure. Dermatologists suggest that you get to know your own skin by doing full body self-exams once each month. If you're worried about something you see... get to the doctor right away. Often patients realize something is wrong. Don't give up if the first physician you see doesn't respond as you like... keep looking until you find one who will take action.

Pioneering doctor remembered for Paralympic idea

LONDON (AP) ? The Olympics have Baron Pierre de Coubertin, the founder of the modern games. The Paralympics have Sir Ludwig Guttmann.

Guttmann, a Jewish neurosurgeon who fled Nazi Germany, pioneered athletic competition as therapy for patients with spinal injuries and organized an archery competition for 16 patients at Britain's Stoke Mandeville hospital in 1948. From this humble start have come the Paralympic Games, which this week will bring more than 4,000 athletes from around the world to London.

"The Guttmann story is massive," said Olympic historian Martin Polley. "He was the one who linked rehab to competitive sport."

This month has been Guttmann's moment, what with a BBC film about his life, "The Best of Men," an exhibition at the Jewish Museum in London and his daughter, Eve Loeffler, being named mayor of the athletes village ? a sort of ambassador in chief who welcomes the participants for games that start Wednesday and end Sept. 9.

It follows a resurgence of interest in Guttmann, who escaped in the late 1930s and settled in Britain, where his research on treating spinal patients drew the attention of the government. Guttmann began working with injured soldiers at Stoke Mandeville hospital, just north of London, during World War II ? a time when suffering a spinal injury was considered a death sentence. Patients were discouraged from moving, leading to secondary infections from bed sores or from pneumonia.

Known for an authoritarian streak and his stubborn insistence on changing the status quo, Guttmann swept into the hospital and took patients off sedation, which had been administered to make them comfortable. But Guttmann was having none of that. No one was going to be comfortable.

"One patient told me 'I'm waiting for God almighty to take me up," the Times of London reported Guttmann as saying at a conference in 1962. "I told him 'While you are waiting, you can do some work'."

He made the patients sit up and work muscles. Seeking to keep them motivated, he hit upon competition as a way to make them work harder. He tried wheelchair field hockey, but when that became too violent, he got the patients involved in wheelchair basketball, Loeffler said.

It was tough and demanding, but grateful patients nicknamed him "Poppa." They were still paralyzed. But many lived ? and carried out Guttmann's wish that they become taxpayers.

Loeffler said her father was marked by his past. Many of his relatives perished in Auschwitz. He was driven, intent to give back to the country that had given his family refuge.

"I think that's another thing that made him work so hard," she said. "He was Hitler's gift to this country in a way, and he was determined to be a good British citizen."

On the same day that London opened its 1948 Olympics, Guttmann organized a competition for wheelchair athletes, which he called the Stoke Mandeville Games. They involved 16 patients taking part in archery. From those humble origins, the Paralympics have grown into a festival of sport involving 4,200 athletes from 166 teams. Though smaller than the Olympics ? which had more than twice the number of athletes ? London's event represents the largest field ever for the games.

The London event ? the first to have come as a result of a joint bid from the host city ? have also benefited from greater integration with the Olympics, particularly in terms of marketing and organization. London essentially connected the rings to the Agitos, the Paralympic symbol of three circling arcs ? a move that helped also spread the fairy dust of attention that the Olympics receive to the Paralympics as well. Queen Elizabeth II agreed to open the event, a reflection of their importance to the country.

Some 2.3 million tickets have been sold, the most ever, many to Britons new to Paralympic sport but who wanted to get an opportunity to see the glittering new stadiums and experience the excitement of first class sporting competition.

Britain's team is also strong. They were second in the Paralympic table at Beijing with 102 total medals, including 42 gold. The hometown crowd wants to see their athletes ? and of course wants to see them win.

Loeffler believes her father's vision has come to pass. And when she wanders the village, people recognize her, like Canadian athlete Jessica Vliegenthart, who saw Loeffler having her photograph taken in a smart navy jacket with its massive circular gold badge.

"Aren't you the daughter of ..." Vliegenthart asked, as two wheelchair basketball athletes drew their wheels in.

Loeffler nodded, her pride clear. Her father dreamed of an "Olympic Games for disabled men and women," and now a whole new generation is learning about him.

"Who but he could have imagined that it would have happened," she said. "I don't think anybody else could."

Britain embraced Guttmann, giving him a knighthood in 1966 and making him a fellow of the Royal Society ? honors of which he was extremely proud. Though he died in 1980, he lived long enough to see the Paralympics grow. The first full scale games took place in Rome in 1960, and souvenirs from the 1964 Tokyo Paralympics are part of the collection on display at the Jewish Museum.

"I just couldn't be more proud of him," Loeffler said. "I just hope he's up there watching it all."

Syria refugees stranded at border half-way point

Terrorised by shelling and air strikes, tens of thousands of Syrians have packed up and crossed the border to Turkey, but some have also been forced to take refuge at the actual crossing.

Hundreds of people are stranded in what was once a bus station on the Syrian side of the Bab al-Salama crossing. Just across the border on the Turkish side, their compatriots are living in a massive refugee camp, complete with tents, satellite dishes and a mosque.

But the camp is now full, and those without a passport and the means to pay their own way in Turkey have found the border crossing bus station is the farthest from the fighting that they can get.

Ali, 34, brought his family from nearby Aazaz five days ago. His three children, aged two, five and seven, play near their father on a mat laid on the pavement where travellers once waited for shuttle buses.

"We came because of the shelling. At first it was bearable, it was fairly light, but after the air strike that demolished several homes I started to change my mind," he said.

His mother, 55-year-old Umm Hassan, her blue eyes wide with anger, described seemingly endless sleepless nights.

"We are not the Free Syrian Army, we are regular citizens who have children and wanted to live in peace," she said.

"We used to hold our children up to the sky to show we are civilians, but the shelling never seemed to stop."

Since the family arrived, they have been trying to find a way across the border, but they do not have passports and have been told that formal refugee camps are full and they will have to wait for more space to open up.

Even in the relative safety of the border Ali feels uncomfortable, worried that the makeshift camp will eventually be targeted by the regime.

"We are waiting for anyone who can get us out of here, to anywhere at all."

Mahmud Khalaf, from the village of Haritan near Aleppo, is sitting nearby on a chequered blanket.

His wife rocks her weeks-old baby in a crib with one hand while she bounces two-year-old Mohammed on her knee.

They arrived a week earlier and have no idea when they might be able to move to a proper camp.

"We just want someone to tell us when we will be able to leave," Khalaf said. "We brought nappies and some baby milk from home, but we are starting to run out, and we don't know where we'll get more from."

The rebel forces who control the Syrian side of the border oversee the area informally, ensuring that theft and assault are not a problem for now.

And they also help to distribute food that comes to the camp from both sides of the border, mostly from Turkey in the form of donations from local or international organisations.

But the camp is not under the formal administration of any body or charity, and the refugees stranded there say they feel abandoned.

"Tell me, should I go back to Marea?" asked a woman who declined to give her name. "I left because of the shelling and the fighting, but here I feel afraid too, and the situation is awful."

A water tanker has been set up on the edge of the bus station-turned-camp, and toilets are available. But there are no showers, food distribution is irregular and chaotic, and privacy is non-existent.

The families have taken over every available part of the pavement, laying out mats or rugs that represent their own claim to personal space.

They have a few belongings piled up on the outskirts of their new homes -- a fan, a bag of potatoes, a handbag or two.

Children run around with little to do. A boy writes graffiti on a wall: "The free city of Aazaz." A girl in a dirty orange dress with a white ribbon trim spins around and around, tracing the edge of her skirt as it flies up.

Even in the shade of the bus station roof the heat is oppressive, and most of those sheltered underneath it are laid out on the floor, trying to keep cool.

Zeinab, 18, her pale face framed by a black headscarf, is trying to soothe six-month-old Amira, her second child.

"We left Marea because we couldn't find anything for the children -- food, medicine," she said. "But we don't have any way to get across the border, and life here is unbearable."
